# CMS Issues Final Rule for ESRD Payment System Changes in 2027

On October 30, 2026, the Centers for Medicare & Medicaid Services announced a final rule updating the End-Stage Renal Disease (ESRD) Prospective Payment System for calendar year 2027. The rule includes updates to the payment rate for renal dialysis services provided by an ESRD facility to individuals with acute kidney injury and revises the requirements for the ESRD Quality Incentive Program. These regulations are effective on January 1, 2027.

#### Medicare Program; CY 2027 Changes to the End-Stage Renal Disease (ESRD) Prospective Payment System, Acute Kidney Injury (AKI) Dialysis Payment, and ESRD Quality Incentive Program

Action: Final rule.

#### Summary

This final rule updates and revises the End-Stage Renal Disease (ESRD) Prospective Payment System for calendar year 2027. This rule also includes updates to the payment rate for renal dialysis services furnished by an ESRD facility to individuals with acute kidney injury. In addition, this rule updates the requirements for the ESRD Quality Incentive Program.

- Takes effect: January 1, 2027
